Schedule: Full time
Shift: Days, M- Friday 8:30am – 5:00pm
Location: Field work – covering Adams, Arapahoe, Jefferson, Denver, Douglas, Elbert and Elizabeth Counties.
The role you’ll contribute:
The Certified Nursing Assistant is responsible for providing personal patient care services to assigned patients on hospice care in private homes or in skilled nursing facilities per the plan of care, in compliance with hospice, State and Federal regulatory and licensure standards.
The value you’ll bring to the team:
·Travels to homes, skilled nursing facilities or assisted living facilities and provides personal care to assigned patients for the requite number of times per week as set forth in the plan of care.
·Provides personal care services to patient including bathing, shampoo, nail and foot care, oral hygiene, assisting with Activities of Daily Living (ADL) and other services as necessary.
·Aids the patient’s mobility and performs range of motion activities on patient extremities as outlined in the Plan of Care.
·Cleans and maintains the patient area. Changes linens and does patient laundry as needed.
·Prepares and serves meals or snacks as directed. Assists patient as needed to eat or drink.
Qualifications
The expertise and experiences you’ll need to succeed:
KNOWLEDGE AND SKILLS REQUIRED:
·Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements.
·Knowledge of electronic medical record.
·Ability to follow standard precautions and uses personal protective equipment when indicated.
·Ability to complete AdventHealth Hospice Care Orientation and Training, as necessary.
EDUCATION AND EXPERIENCE REQUIRED:
·The successful completion of the Certified Nursing Assistant curriculum.
EDUCATION AND EXPERIENCE PREFERRED:
·High school diploma or equivalent
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
·A clear/active Certified Nursing Assistant License for the state operations are provided.
·Maintain a current, valid Driver's License in good standing with a safe driving record and a reliable vehicle.
·Maintain valid personal automobile insurance.
·Basic Life Support Certification
LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:
·Certified Hospice and Palliative Nurse Assistant (CHPNA)
