BJC HealthCare System is hiring a Full-TimeMSW Social Worker (Case Management) – ED Dept for Christian Hospital NorthEast.
Position Details:
Day Shift Hours: 8am-4:30pm (Mon-Fri) with some Saturdays, Holidays, and On Call
ED Team - Role is in the emergency department to address immediate biopsychosocial needs and provide crisis intervention for patients and families who present to the emergency department
Completing biopsychosocial assessments, Grief support and funeral resources, Connect patients with specialty care, Referring patients with complex medical needs, Advocacy work with Ombudsman, Scheduling medical follow ups
Victims of violence support, patients needing immediate nursing home placement, Children needing emergency shelter/services due to the illness of the primary caregiver, Abuse and/or neglect reporting for children and adults, Emergency placement for adults and minors, addressing complex family dynamic issues, assistance in locating family or healthcare surrogate, Assisting with avoidable ED Admissions
Experience:
Preferred: 2-5 years of work experience
ability to work independently
Experience in health care and/or behavioral health
Requirements:
Master's Degree of Social Work (MSW) – required

Northwest HealthCare, six miles west of Christian, offers 24- hour emergency care, the Sleep Disorders Center and a variety of outpatient diagnostic and imaging services.
The Case Management Department provides services through a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's needs. The Case Management Department is responsible for all acute care inpatients where needs have been identified or who have requested assistance, emergency department patients where needs have been identified, and community individuals who request assistance.

Role Purpose
Under direction of a physician, assists patients and their families in their residences or outpatient/inpatient settings to adapt and plan an interdisciplinary approach to treating the patient-family system.Employees working at an Illinois facility are required to have an Illinois state social work license or be able to take and pass the Illinois state social work licensing exam within 90 days of the first day of employment.
Responsibilities
Provides direct clinical services to patients and families.
Plans and implements social work services on assigned clinical service or unit in collaboration with the healthcare team.
Collaborates with community human service agencies to meet the needs of patients, families, and the hospital.
