Job Summary
Under general supervision, the Care Management (CMA) position will support the Care Management Department in providing paraprofessional support for discharge planning. In the discharge planning process, the CMA will communicate with and coordinate post discharge services as directed collaboratively by the Care Management Nurse Care Coordinators and the Clinical Social Workers. Additionally, the CMA will perform general office administrative duties as required, and cover for the URRN CMA in their absence.
Essential Functions:
Manages appropriate transitions of care for patients regardless of payor from ED (Emergency Department), PAV C and inpatient via collaboratively coordinating with Nurse Care Coordinators (RNCC) and Clinical Social Worker (CSW) using a "customer service" framework to facilitate timely discharges through communication and referrals to post discharge providers including home health, home and inpatient hospice, long term acute care facilities, skilled nursing facilities, assisted living facilities, transportation, and any other type of post discharge provider of services (20%)
Serves as a contact for external providers and conducts follow-up phone calls, as needed to confirm referral receipt and obtain disposition (approval or rejection) regarding post discharge referrals and maintains ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety. (20%)
Conducts research to identify providers for specific services as needed by the RNCC and SW for the purpose of reviewing the organizations to determine their partnership with insurance companies, to identify cultural specialties and limitations and to meet the unique discharge needs of each patient. (15%)
Follows the organization?s release of information policies and procedures and identified communication system (Epic fax, fax, email, US mail, etc.) to electronically send the pertinent information for each referral type as directed. (20%)
Schedules follow-up appointment, as requested, to support post-discharge follow-care including physician office visits, wound care clinic visits, infusion center services, etc. (15%)

Education:
Bachelor's Degree social science or health care field Required or
High School Diploma or GED Required and
Four years in experience in healthcare related fields such as hospital, home health provider, insurance utilization review department, doctor?s office, case management, DME company, etc. Required
Work Experience:
1-3 years Experience in healthcare related fields such as hospital, home health provider, insurance utilization review department, doctor?s office, case management, DME company, etc. Required
Knowledge, Skills and Abilities:
Knowledge and expertise in developing and using data management tools such as Microsoft, Epic, and experience with office machines (ie- copier, fax, telephone), computers, software (ie- word processing, spreadsheet, etc.).
Expertise and knowledge of third-party payor requirements and regulations including Medicare/Medicaid, managed care payors, commercial insurance companies, worker?s comp, etc.
Ability to work within strict time limits when responding to verbal and written requests for review.
Must be flexible and well organized and able to prioritize.
Sound knowledge of medical terminology.
Skilled in establishing and maintaining cooperative and "team" oriented working relationships with others.
Highly effective communication skills, both verbal and written.
Strong organization, time management and follow-through skills.
Solid problem solving and decision-making skills. Demonstrates flexibility and assertiveness.
