SUMMARY OF POSITION:
Provide individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family caregivers. Provide patients with information and counseling, and make referrals for other services. May also provide case and care management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.
It is a great opportunity to work with a unique patient population, transplant, which is highly rewarding. The social worker works with these patients and their families from time of referral into the transplant program, up until transplant and then for the remaining lifetime of that patient which allows the social worker to develop strong bonds and relationships with the patients and families that entrust them with their care. This position allows for independence and flexibility while managing day to day patient care. It is a Monday-Friday, typically 8 hour days/week with no weekends or holidays. There are many opportunities for professional growth in this position by allowing for attendance at transplant related conferences, input and creation of day to day workflows and processes and participation in a multitude of multidisciplinary venues (Quality, Education, Performance Improvement, etc.). There is an opportunity to work and collaborate with many people through out the organization that contribute to the program (Inpatient and Outpatient Leadership, Inpatient Social Work and Care Coordination, Physicians, Surgeons, OR Teams, Pharmacy, Nutrition, Psych, Nurses, Coordinators, Quality Associates, Financial Coordinators, Administrative support teams, Outreach Coordinator, Educator, etc.).
