Job Summary: We are seeking a dedicated and detail-oriented Licensed Vocational Nurse (LVN) to join our remote team of nurses. Must be able to work independently and as a part of the remote team. The LVN will manage specialty medication refills and assist with in-basket tasks for various specialties, including Pain Management, Neurology, Pulmonary, Infectious Disease, Urology, Urogynecology, and Nephrology. The role also involves handling live phone calls for patient support and coordination. Additionally, LVN may need to cover in-person clinic requests when required. When directed, attend mandatory staff meetings, EDU, RQI, and precept new staff in person.
Minimum of 2 years of nursing experience, preferably in a specialty area.
Ability to work independently and as a part of a remote team.
Excellent communication and interpersonal skills.
Assist with in-basket management for Pain Management, Neurology, Pulmonary, Infectious Disease, Urology, Urogynecology, and Nephrology.
Handle live phone calls professionally and appropriately.
Cover in-person clinic requests when needed if staffing is required.
Collaborate with the remote nursing team to ensure seamless communication and workflow.
You will use our nursing model to care for patients as part of our health team. Your role involves tasks based on your license, including managing patient electronic medical records. Handle digital interactions through our 'In Basket' system.
Essential Functions of the Role
Respond to patient messages promptly, adhering to standard workflows.
Act as the first reviewer for all assigned care team messages.
Carry out fundamental scheduling tasks which include appointments for the same and next day, as well as lab rechecks.
Take charge of referrals and pre-authorizations as per clinic workflow.
Fill out necessary paperwork such as Family and Medical Leave Act documents, home health records and others.
Implement standing delegated orders efficiently.
Handle pending/plan orders and associated diagnostic codes.
Ensure clear and quick documentation in the electronic medical record.
Record external test results in the electronic medical record.
Make patient calls and notifications, ensuring accurate relaying of clinical information/education from provider to patient.

Qualifications
You must have completed a high school education or equivalent.
We require a minimum of (1) one year experience.
(RN) Registered Nurse license.
Basic life support (BLS) certification is required. Must obtain within 30 days of hire if you do not have it.
