Job SummaryCare Coordinator, LVN (CC-LVN) manages requests for Care Transitions & Integration by communicating with internal and external entities to ensure referrals are being completed. Facilitates authorization procurement for third party payers by providing pertinent clinical documentation according to department guidelines. Facilitates care coordination for complex patient care needs and assists with outpatient procedures and placement as indicated.
