The Licensed Practical Nurse is a graduate of an accredited school of nursing and is licensed to provide safe patient care as well as highly skilled nursing care under the supervision of a Physician and/or Registered Nurse. The Licensed Practical Nurse provides a higher level of evidenced-based practice as per the Nurse Practice Act and other governing bodies and demonstrates strong leadership and efficient skills in caring for patients with chronic diseases; requires consultation with advanced practice clinicians and benefits from feedback; reports directly to the Director of Nursing.
ESSENTIAL FUNCTIONS
Triage all patients who present with medical emergencies and determine their medical state and follow-up with the medical Providers accordingly.
Triage patients who are scheduled for routine visits.
Coordinates the day-to-day clinical patient flow ensuring patient safety and providing solutions to any challenges and/or obstacles within the clinical team.
Travels from site to site, when necessary, to provide direct patient care, triage, and/or other duties as assigned.
Conducts health/educational assessments and re-evaluations during intake and conducts all components of the initial triage process.
Acts as the patient advocate and consults with the physicians where necessary.
Assist with the coordination of resources with the interdisciplinary team.
Assist the physicians with individual health education as directed and provides literature accordingly.
Coordinate appointments for the physicians and the Pharmaceutical Representatives.
Assist with the development of policies, procedures, and clinical protocols.
Participate in case conferences with the interdisciplinary team and provide supporting facts where necessary
Conducts monthly quality assurance on all medical equipment as assigned.
Maintains appropriate par levels on all injectable medications and administer.
Orders all medical supplies.
Orders office supplies for medical team.
Prepares monthly, quarterly, and annual reports in a timely manner.
Encourages professional development to other Licensed Practical Nurses, Medical Assistants, Laboratory Technicians and other auxiliary team members.
Foster positive, professional intra-agency, inter-agency relationships.
Assists with the development of orientation for new employees and serves as the nurse preceptor.
Completes the monthly clinical schedule and submit to the Nurse Manager as directed.
Assist the Director of Nursing with administrative duties as assigned.
Empower clients to make healthy decisions in an effort to improve the quality of lives.
Maintain patient files in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
Process routine, urgent and Stat referrals.
Understand insurance process and requirements.
Attend team meetings and provide clinical updates where necessary.
Obtain Mediquire reports one day prior to prepare for each next morning huddle to look for gaps in care.
Perform other duties as assigned.

EDUCATION AND EXPERIENCE
Must be licensed to practice in the state of Virginia.
Must have current CPR (Healthcare Provider) certification.
Minimum of three years of nursing experience.
Must have a valid Virginia Driver’s License, satisfactory driving record, and transportation to travel to our various medical offices.
