Job Summary
This is a grant- funded position with job responsibilities set forth by grant contract statement of work. The Assistant Case Coordinator serves as an intermediary between the Denver Sheriff's Department, social services, and community partners to facilitate access to services in support of criminal offenders incarcerated in Denver jail. The Assistant Case Coordinator assists the patient in obtaining the care and resources needed to maintain Medicated Assisted Treatment (MAT); identifying patient needs to reduce barriers to timely treatment, and connecting patients to sources of clinical, financial, or social support in the community. Under minimal supervision provides navigation services as it relates to outreach, care coordination and referral management.
Essential Functions:
Provides outreach and administrative case management to patients in the jail. (10%)
Perform any administrative and clerical duties as it relates to patient care-coordination and referral management. (10%)
Liaison between jail-based health care providers and patients. (10%)
Assist clients with scheduling outpatient appointments and transportation needs. (10%)
Maintains reports, provide necessary documents and data by deadlines upon request. (10%)
Educate clients about the importance of routine after-care in the community, ensuring clients are referred to an appropriate community provider, and coordinating referrals and facilitating patient engagement in treatment. (10%)
Coordinate patient care collaboratively with community partners to ensure better service delivery and education to the population served. (10%)
Provide clients with lists and referrals to appropriate providers. (5%)
Conduct post-release follow-ups with patients to assess progress and engagement in treatment. (5%)
Follow-up with members who missed their appointment as requested by physical, oral and mental health providers. (5%)
Attend meetings, training, seminars, or conferences as required to keep current on continuing education requirements with DSD and Denver Health. (5%)
Maintain a current list of health care providers and collateral resources for individual client use and to share with community partners by phone call and face to face interactions. (5%)
Ability to develop effective therapeutic interventions that are socio-culturally sensitive. (5%)
Education:
High School Diploma or GED Required
Work Experience:
1-3 years Two years of clerical experience, preferably in a managed care or medical setting or relevant customer service experience Required and
Direct care experience with homeless and/or substance abuse populations, preferred. and
Previous case management duties in areas of long-term, ongoing support and linking to other resources, preferred.
Licenses:
DLR-Colorado Driver's License Class R - DMV - Department of Motor Vehicles Required and
BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association Required
Knowledge, Skills and Abilities:
Ability to work flexible hours.
Excellent communication, organization and people skills.
Creative problem solving abilities.
Demonstrated ability as a self-starter who can prioritize and conduct multiple activities.
Understanding of basic care coordination and case management-related service functions (patient interviews, discharge planning, referrals, community resources, finance, and insurance/payer communications).
Bilingual English/Spanish, preferred.
Communicate effectively in a written and verbal form with health care team, patients/families and community, internal and external providers.
Flexibility in responding to multiple demands.
Ability to work autonomously with minimal direction from supervisor.
Basic knowledge of Microsoft applications and email.
