As a member of the broader Home and Community Care team, you’ll provide annual clinical assessments to patients in the comfort of their homes. The Clinical Advisor is a field practitioner who provides mentoring, coaching, and support for nurse practitioners (NPS) in the field. The clinical advisor partners with the Clinical Team Managers (CTM) to enhance clinical expertise and adherence to the HouseCalls' clinical model.

Primary Responsibilities:
Coaching and Evaluation of NPs with the HouseCalls visit - over 50%
Partner with CTMs to ensure clinical practice adheres to HouseCalls’ clinical model
Utilize advanced clinical nursing expertise, knowledge of geriatric/chronic disease management to provide coaching, mentoring and role-modeling to new and existing NPs
Identify and support developmental clinical needs of NPs to achieve successful outcomes
Ensure appropriate implementation and adoption of clinical and quality initiatives as related to home visits
Foster and develop a culture of clinical expertise
Provide visit feedback to NPs
Provide a summary of the visit to the CTM
Works with Regional Director and CTM to determine percent of time in the field completing member visits
Perform additional duties as identified by the business needs

Required Qualifications:
FNP, GNP, ANP, board certified or eligible with an active, unencumbered licensure free any open/unresolved disciplinary actions including probation or restrictions against privilege to practice
3+ years of experience in a clinical setting
1+ years of field experience as a HouseCalls NP
Ability to exceed performance metrics
Ability to pass QA consistently
Evaluation score (Ride Along) of 90% or above
No valid member complaints on record
Ability to travel to assigned locations
