Join BJC Health System in providing Wellness Coaching and Outreach for members of an employee sponsored health plan. Provide wellness coaching as well as outreach for patient assessment and care gap closure including reaching out to members for assistance in scheduling appointments for care gaps.
You will also attend employer biometric screenings to welcome employees and provide education on the benefits of wellness coaching.
Schedule
Day hours with at least 2 days per week with evening hours.
Weekends based on client needs.
Hybrid position supporting metro St. Louis and outlying areas.
Requirements
Health and Wellness Coach certification at time of start

Role Purpose
The Wellness Coach is responsible for partnering with identified clients to provide health and wellness coaching. This position utilizes a collaborative process of assessment, planning, implementation, and evaluation to engage, educate, and promote the client?s decisions related to achieving and maintaining optimal health status. This position will also serve as a resource towards enhancing the well-being program.
Responsibilities
Establishes client-centered relationships and serves as a facilitative partner to help clients assess readiness for change, determine focus, set goals, prepare for action, and maintain behavior change.
Incorporates skills, such as active listening and presence, motivational interviewing, reflection, and SMART goal-setting to assist client in making change.
Performs assessment of clients through the use of clinical tools and information/data review; conducts comprehensive evaluation of member?s needs and benefit plan eligibility for available integrated internal and external programs & services.
Develops, executes, and provides support for well-being programs & initiatives.
On behalf of the health plan, completes general outreach to members to support care compliance with recommended guidelines and program goals.
