Responsible for serving as the first reliable point of contact for Patients, Families and Providers.


Principal Accountabilities:


1. Serves as patient and family advocate for liver patients.
2. Coordinates the scheduling of appointments to expedite the plan of care and to promote continuity of care.
3. Schedules, re-schedules, and cancels appointments for the appropriate outpatient service.
4. Assists in education of the patient/family to understand disease process and care map.
5. Assists in answering patient calls, triaging patient calls, initiate call backs and follow-up to meet the needs of the patient.

6. Assists with communication to internal departments and external contacts for navigating the Liver Transplant partnership with NYU Lagone.
7. Assists with communication to internal departments and external contacts for insurance verification and authorization process.
8. Supports a smooth transition of patients from outside referrals to internal referrals for appropriate treatment.
9. Performs other related duties as assigned.
