Senior Focused Nurse Practitioner / Physician Assistant Opportunity in Springfield, MO!

Mercy Clinic Internal Medicine-Smith Glynn Callaway is recruiting Advanced Practice Providers with an interest in building relationships, caring for patients with complex needs, changing lives, and making a difference for seniors. This is an outpatient-only position with patients on Medicare and Medicare Advantage.

• In-person, virtual, or telephonic encounters - annual comprehensive visits, transitional care, and same day visits

• Provide team-based care to manage a panel of 1,000 with collaborating physician allowing for longitudinal relationships with providers, patients and families
• No holidays, weekends, or evenings
