Job DescriptionJOB SUMMARY: Under the supervision of the director, assist the in-patient high risk care management team by coordinating with all disciplines throughout the hospital to ensure smooth care transitions. Responsible for maintaining hospital compliance with a variety of documentation, including Electronic Discharge. In order to reduce readmissions to the hospital, make appointments for High Risk patients to see either their Primary Care Physicians or Specialist Physicians.
