Provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice.
Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment. Documents patient care given. Monitors, records, and communicates patient condition as appropriate to care team, physician, patient, and family.
Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation. Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs. Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances. Works as an advocate for the physical and emotional well-being of the patient.
Duties and Responsibilities:
?Makes rounds with outgoing shift; knows whereabouts of assigned patients. ?Makes rounds with oncoming shift to ensure the unit is left in good condition. ?Documentation meets current standards and policies. ?Provide daily socialization activities as scheduled, to specifically include daily Reality Orientation. Document and encourage patients to participate. ?Observes patients? behavior and mental and physical conditions Reports any changes and unusual incidents observed to supervising Nursing Staff ?Knowledgeable of sterile techniques, infection control procedures. ?Knowledgeable of environmental risks for suicide and response process to follow in in the event of a suicide. ?Participates in and contributes to Patients? Treatment Plan and Patient Care Conferences.
Non-Essential Functions: ?Checks unit for adequate linen. Folds neatly and arranges linen in linen closet. Cleans linen cart. Provides clean linen and clothing. Makes beds. Minor housekeeping. ?Ensures an adequate stock of supplies and proper functioning of equipment.
Qualifications
Qualifications: Education: High School Diploma or General Equivalency Diploma (GED), or equivalent, is required
Required Certifications: ?American Heart Association Basic Life Support (BLS) Certification required within 90 days of hire or transfer. ?Handle With Care Behavior Management System (HWC) Certification required within 90 days of hire or transfer. ?Blood Glucose Monitoring Training Program Certification required within 90 days of hire or transfer.
