BJC HealthCare System is hiring a Full-Time Social Worker (MSW) Intake Coordinator for our Medical Stabilization Serviceslocated at the Parkland Health Center (Farmington, MO)
Position Details:
Day Shift Hours- M-F 8:30am-5pm (hours may vary)
Rotating weekend on call
Coordinate intake and admitting procedures for chemical dependency services
Ensures referral source satisfaction
Provides evaluation documentation utilizing available data
Link patients to treatment and ensure smooth transition to the next level of care.
Provide support for supplementary social work requirements and/or tasks related to discharge planning
Experience
Social Work experience in Chemical Dependency and/or Emergency/Inpatient Medical Care (preferred)
Requirements:
Master's Degree of Social Work (MSW) – required
Overview
As part of BJC HealthCare, Parkland Health Center offers a unique combination of world-class medicine along with the deep local roots and rich history of its three predecessors – Bonne Terre Hospital (opened in 1911), Mineral Area Regional Medical Center (1952), and Farmington Community Hospital (1969). Parkland Health Center has two locations in St. Francois County, Missouri – a full-service acute care hospital in Farmington as well as a rural emergency hospital in Bonne Terre. Parkland Health Center provides quality primary and secondary level inpatient and outpatient services to the people of St. Francois County and surrounding areas in southeastern Missouri.
In addition, Parkland’s Farmington campus offers a full continuum of inpatient and medical-surgical and intensive care, obstetrics, geriatric psychiatry and emergency services, along with a wide variety of outpatient services including oncology services, an infusion center, a wound care clinic, and the Huckstep Heart and Lung Center. Bonne Terre is a Rural Emergency Hospital providing emergency services and outpatient services including medical observation, lab, radiology, and sleep study.
The Care Coordination Department provides support for the accurate and timely processing of patient care at PHC. This care includes finding the highest quality discharge planning possibly within our local community.
Preferred Qualifications
Role Purpose
Under direction of a physician, assists patients and their families in their residences or outpatient/inpatient settings to adapt and plan an interdisciplinary approach to treating the patient-family system.Employees working at an Illinois facility are required to have an Illinois state social work license or be able to take and pass the Illinois state social work licensing exam within 90 days of the first day of employment.
Responsibilities
Provides direct clinical services to patients and families.
Plans and implements social work services on assigned clinical service or unit in collaboration with the healthcare team.
Collaborates with community human service agencies to meet the needs of patients, families, and the hospital.
