Overview
The Behavioral Health Complex Case Manager is responsible for assisting patients and families in managing mental health, substance use and psychosocial issues by developing disposition and execution of discharge plan, with a focus on long length of stay and complex patients. Serves as a patient/family advocate and a communication link between the physician, patient, family, health care team, community resources, and payers to reduce length of stay and recidivism. Anticipates potential delays in the disposition and acts pro-actively to avoid these delays. Functions as a resource and placement specialist for the team with strong working knowledge of MH/SU levels of care. Collaborates with both inpatient/outpatient community partners and the Network for Advancing Behavioral Health for successful continuum of care. Will conduct safety assessments; arrange transportation, SDOH migration and patient/family education as needed. The Behavioral Health Complex Case Manager's caseload will include (but is not limited to): all Behavioral Health patients (pediatric and adult) who are medically cleared, have a length of stay of 48 hours+ and are considered complex. This includes patients identified as frequent readmits with/without community services, do not have a viable discharge plan, Behavioral Health patients who require a significant amount of time to develop a discharge plan and/or any other patients as assigned by BH Supervisor/Manager. When needed, the Behavioral Health Complex Case Manager will develop and monitor patient-centered behavioral health plans and will work with families who would benefit from more in-depth conversation/meetings. The Behavioral Health Complex Case Manager with be the lead for department-specific projects and other tasks/projects as approved and assigned by BH Supervisor/Manager. During periods of increased patient volume/surge, the Behavioral Health Complex Case Manager will be utilized for staffing as deemed necessary by Behavioral Health Supervisor. The Behavioral Health Complex Case Manager will work as part of the Behavioral Health team, will participate in daily huddles, staff meetings and provider meetings. The Behavioral Health Complex Case Manager will report to Behavioral Health Supervisor.
