AdventHealth Winter Park is a hospital built for the community by the community. The land for the hospital was donated by the city of Winter Park.
Chartered in 1951, the hospital is a 320-bed acute care facility that is a model of community health and wellness, and a teaching hospital. We encourage our team members to grow professionally by providing educational opportunities, and resources.
We provide the most advanced care available with upscale and boutique care for Obstetrics, Level II Neonatal Intensive Care Unit, a state-of-the-art Emergency Room, comprehensive Cancer Care, advanced technology Surgical Services with 12 surgical specialties, a first-of-its-kind Inpatient Rehabilitation featuring high-tech training equipment, Surgical Orthopedic destination with a Dedicated Education unit, and Stroke and Ortho DNV Certified, to heal the community in the most comfortable way possible.
The city of Winter Park is filled with rich heritage and is located just three miles north of Orlando in Orange County. The city is known for its old-world charm, quaint bricked streets and is a nature lovers' destination. It’s main street, called Park Avenue includes civic buildings, art galleries, a private liberal arts college, museums, parks, golf country clubs, boat launches and more.
The role you’ll contribute:
The Discharge Coordinator makes post discharge phone calls to both inpatients and Emergency Department patients, answers general questions patients may have, and receives feedback regarding their patient experience. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
The value you’ll bring to the team:
Makes telephone contact with patients within 24-48 hours after discharge to assess patient education, compliance with discharge instructions, and patient satisfaction.
Serves as a liaison between discharged patients and professional staff and clinical departments. Answers questions regarding discharge instructions and uses available resources to provide immediate service recovery.
Uses a centralized database to record and track patients’ responses. Supports the clinical managers in recognizing employees deserving recognition as they are identified in the discharge telephone process and performance improvement.
Intervenes promptly when issues are reported after discharge. Makes recommendations for both process and performance improvement.
Uses key words and phrases to strengthen the overall impression of AH. Serves as a role model for the AH patient experience.
The expertise and experiences you’ll need to succeed:
Minimum qualifications:
Associate’s degree in Nursing
Minimum of three years’ experience with direct patient care: inpatient, acute
Current valid State of Florida or multistate license as a Registered Nurse
Preferred qualifications:
Bachelor’s degree of Nursing (BSN)
Experience in risk management, discharge planning or complaint management
