Duties

• This is a Temp position NTE 1 year
• The Advanced Practice Nurse (APN) Primary Care Nurse Practitioner Residency (PC-NPR) Program Director (PD) practices in Nursing - Clinical Operations and Primary Care Service to provide a full range of services emphasizing quality - timeliness and advancing the Veterans Affairs (VA) mission
• The PC-NPR PD executes leadership characterized by substantial and continuous responsibility and accountability for population groups or integrated programs that cross services and/or disciplines and influence the org PC-NPR PD practices to the full extent of their education - training and certification
• Major duties and responsibilities of the position: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
• Develops and implements an individualized plan to achieve optimal population health and disease management
• Demonstrates leadership and works collaboratively with the interdisciplinary team
• Orders and interprets labs - tests and diagnostic studies
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals and consults to specialty services
• Provides education to patients and their families
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

• Provides input in the budget - mission - operations and development of the PC-NPR program
• Utilizes current trends and relevant theories in a analytical framework to design - modify - implement and measure the PC-NPR program and activities to achieve clinical and administrative goals
