The role you’ll contribute:
Under the direction of the Health Management leadership team, the Clinical Operations Manager role provides supervision and leadership to a virtual interdisciplinary team, including recruitment, onboarding, orientation, education, and performance management of assigned direct reports. Supports the organizational mission and operational goals of the Health Management Team by routinely analyzing and interpreting all available data and reporting, maintaining daily staffing standards and productivity expectations, while routinely monitoring/auditing direct reports for production of high-quality, person-centered, collaborative work. Evaluates current processes to determine appropriate recommendations and updates. Responsible for ensuring quality interventions that result in the delivery of best-in-class case management and care coordination outcomes including maintaining compliance with regulatory requirements. Attends pertinent seminars, workshops and professional meetings to maintain relevant knowledge regarding population health management and leadership trends. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Frequently reviews and trends available data and reports in order to performance manage direct reports in a timely and supportive manner
Maintain a positive work environment that influences efficiency and attainment of service metrics and key performance indicator goals.
Manages program and direct reports in a manner that meets applicable regulatory licensure and accrediting regulations.
Responsible for implementation of new processes within the department, evaluates current processes to determine process improvement plans and quality assurance
Provides oversight, accountability, and support to achieve expected performance and productivity.
The expertise and experiences you’ll need to succeed:
Minimum qualifications:
Bachelor's Degree in Nursing or LCSW/LMHT
5 years of Population Health, Care Coordination, Transitions of Care, and/or Case Management experience
2 years leading a Case Management, Disease Management, Care Coordination, or Transitions of Care team
Registered Nurse - State Licensure and/or Compact State Licensure or License Clinical Social Worker or Licensed Mental Health Therapist
Preferred qualifications:
A varied clinical background including managed care, health promotion, or population health
Previous experience working with interdisciplinary teams
Previous experience as a telecommuter/member of a virtual team
Certified Case Manager (CCM)
