The role you’ll contribute:
The Outpatient Diabetes Educator, under general guidance, is responsible for providing age specific diabetes self-management education to patients and healthcare professionals. Provide diabetes education services in a new core delivery model as a health coach for corporate organizations and their employees. Actively participates in outstanding customer service and accepts responsibility for maintaining relationships that are equally respectful to all. Adheres to the AdventHealth Corporate Compliance Plan and to all rules and regulations of applicable local, state and federal agencies and regulatory bodies. The value you’ll bring to the team:
Provides self-management instruction in group and individual sessions, in accordance with patient education standards set by the American Diabetes Association, departmental policy through effective application of teaching-learning strategies. Provides assessment and education in all content areas except individualized meal plan calculation for RN or insulin administration for RD. Includes all required documentation and follow-up. Incorporates special needs considerations when applicable.
Establishes individualized learning goals, objectives and educational plan in collaboration with the patient.
Evaluates patient upon completion of educational plan and provides remediation as indicated.
Provides ongoing health coaching utilizing only approved resources to effectively engage the client.
Actively participates and accepts responsibility for special projects such as risk testing, speaker requests, support groups and other community events designed to increase diabetes awareness.
Participates in healthcare professional education programs as assigned.
Performs other duties as assigned or directed to ensure the smooth operation of the department. Orients new staff as necessary.
The expertise and experiences you’ll need to succeed:
Minimum qualifications:
Associate’s degree in Nursing or Bachelor's degree in Dietetics
Active State of Florida Nursing License or State of Florida Registered Dietitian
Current Basic Life Support (BLS) certification
At least one (1) year of experience as a Nurse or Dietitian
Knowledge of diabetes of diabetes concepts and skills
Ability to provide patient teaching in individual and group settings
Preferred qualifications:
Bachelor's degree in Nursing or Master’s degree in Dietetics
Certified Diabetes Educator (CDCES) certification
One (1) year of experience in Diabetes Education
Advanced knowledge of Diabetes
Ability to provide teaching in all aspects of diabetes management
Bilingual in Spanish
