The APP leverages knowledge of neurological testing and imaging, acute stroke treatment plans, and neurovascular intervention. The APP demonstrates strong leadership, is an excellent communicator, and is effectively able to prioritize a large number of patients in various stages of stroke. Job Duties: Works autonomously providing direct patient care including monitoring and evaluating high risk stroke patients. Provides stroke patient care management including assessments and documentation, consultations, and daily patient rounds. Prepares and analyzes clinical data and collaborates with health care team to make diagnoses of acute and complex chronic conditions. Partners with physician appropriately regarding patient care issues and concerns. Evaluates operations from a workflow perspective and responds appropriately, redesigns per patient needs and competitive benchmarks. Leads in the evaluation, development, and implementation of comprehensive patient education programs. Maintains knowledge of current practices in the management of stroke and other neurological disorders and complexities. Participates in professional organizations in stroke management and neurovascular intervention, and other areas of professional interest. Serves as a resource person for staff caring for the stroke population. Assures that the program is in compliance with the appropriate governmental agencies. Develops and presents information to community and professional groups on stroke management. Compiles information for the organization as requested. Provides patient status reports to the community and referring physicians. Develops with the patient, caregivers and patient care team, a mutually acceptable cost-aware plan of care that maximizes health potential. Formulates a treatment plan which includes ordering diagnostic tests, selecting appropriate pharmacologic and non-pharmacologic interventions, identifying educational needs of the patient and family, developing a patient education plan and formulating a discharge/follow-up plan of care. Minimum Requirements: Masters Degree in Nursing Licensed by the State of Missouri as an Advanced Practice Registered Nurse Basic Life Support (BLS) from American Heart Association (AHA) required. Stroke Certified Registered Nurse (SCRN) and National Institutes of Health Stroke Scale (NIHSS) credentials preferred. Proficiency with electronic health records and database management.
