Social Worker
The incumbent will function as part of the interdisciplinary treatment team at the WJB Dorn VA Medical Center within the Home Based Primary Care (HBPC) Program. The HBPC Social Worker provides and coordinates health care services to Veterans in a home environment, residential care setting or virtual video connect (VVC). The incumbent will coordinate linkage with appropriate VA and community service providers or agencies as needed by the client. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
