The role you’ll contribute:
The Joint Replacement Program Coordinator is responsible for the development, coordination, and maintenance of the care continuum for total joint replacement patients.This position provides leadership for all aspects of the continuum of care beginning before surgery through one year postop; including PSC, Perioperative services, inpatient units, post-acute care and all other applicable entities within the orthopedic service line related to evidence-based practice for orthopedic patients.The Joint Replacement Program Coordinator will plan, develop, and implement program goals and operations.This position coordinates and works with infection prevention and quality to continually improve care for the orthopedic population.The Joint Replacement Coordinator will develop, coordinate, and implement the clinical and business activities as well as the evaluation of patient outcomes through chart audits, database entries, and reports. This position will work collaboratively with physicians, post-acute care providers, families, and staff to develop and maintain a multidisciplinary Joint Replacement Program. Responsible for reaching total joint center of excellence and maintain the status.
The value you’ll bring to the team:
·Coordinates the efforts between the surgeons, leadership, and staff while tracking the progress of the program and ID areas requiring attention.
·Responsible for all program management, report generation and review, process improvement,
·Works with orthopedic surgeons and anesthesiologist to manage the patient’s specialized needs.
·Develop policies and procedures for best evidenced based practice for total joints.
·Coordinates the interdisciplinary approach to providing continuity of care, including utilization management, discharge coordination and planning, and obtaining all authorizations.
Qualifications
The expertise and experiences you’ll need to succeed:
Kansas Registered Nurse License
BCLS obtained through the American Heart Association
Bachelor’s Degree in Nursing
Evidence of project management and/or process improvement
1 to 2 years of orthopedic related experience
