The Population Health Pharmacy Program supports Population Health initiatives to increase the quality of care, while maintaining cost-effective outcomes through disease and medication management programs. The Clinical Pharmacist will work closely with the Pharmacy Supervisor and Office of Population Health & Accountable Care (OPHAC) leadership to execute strategies for the Population Health Pharmacy Programs as it relates to value-based care incentive programs. The Clinical Pharmacist will perform a range of functions that include identifying gaps in care, high-risk medications, drug interactions, duplications in therapies, and will analyze pharmaceutical utilization and related quality measure performance.
The Clinical Pharmacist will be a key player in the executing Population Health programs to optimize medication adherence, medication therapy management, and comprehensive medication management. The Population Health Pharmacist will monitor and coach patients on medication adherence and disease management to ensure health goals are met. A primary goal of this role is to enhance performance on population health measures related to medication management. Performance will be tracked by pharmaceutical STAR quality ratings. Other duties include precepting pharmacy students, performing patient education using techniques in motivational interviewing, evaluate barriers to compliance, suggest positive resolutions, identify therapeutic endpoints, and perform other tasks based on population health initiatives to improve patient outcomes. The Clinical Pharmacist will work directly with Pharmacy Supervisor and OPHAC leadership, in collaboration with other internal and external stakeholders.
Salary Range: $83.37-$97.65/hourly
Qualifications
Required:
Current California Board of Pharmacy Pharmacist licensure in good standing.
PharmD degree.
PGY1 Residency Training or equivalent clinical experience
Board Certification in Ambulatory Care Pharmacy (BCACP), Pharmacotherapy Specialists (BCPS), or applicable specialty area certification
Previous experience providing pharmaceutical care in desired specialty and primary care areas
Prior experience in population health or managed care equivalent medication adherence/management programs
Experience with Epic EHR functionality
Ability to utilize Word, Excel and other software programs
Ability to establish and maintain cooperative and collaborative working relationships with administrative, clinical and academic personnel
Skill in working independently and following through with minimum direction
Ability to set priorities which accurately reflect the relative importance of job responsibilities.
Ability to provide evidence-based recommendations for symptom management.
Demonstrate writing skills to compose logical, detailedand grammatically correct correspondence
Document patient care in an concise and comprehensivemanner
Knowledge of efficient interviewing practices and techniques used in order to gather information.
Strong interpersonal skills with the ability to build relationships with providers and patients.
Excellent verbal and listening skills both in-person and on the phone required.
Ability to maintain quality, safety and/or infection control standards.

Knowledge of medical records and appropriate documentation requirements.
