In thiskey role, you will be responsible for rotatingshifts and areas with a primary focus in oncology and CAR-T program patients.You will preparation and dispense drugs in all inpatient settings; maintain andreview patient medication profiles; review and verify physician medicationorders and patient medical record. You will interact with healthcareprofessionals and providers to help promote safe medication use, preventinappropriate or misuse of medications; provide clinical pharmacy services(e.g., pharmacokinetic dosing, therapeutic interchange, IV/PO conversions, TPNper pharmacy, clinical protocols per pharmacy and pharmacist interventions.When assigned to decentralized duties in patient care and Oncology units, youwill be physically located in the patient care areas and primarily responsiblefor verification of new admission medication orders, medication history takingand reconciliation of high risk patients upon admission, and rounding withMD/RNs beginning on admission to provide effective medication therapy duringthe patient's stay and early planning for discharge.

HourlySalary Range: $83.37 - $97.65 (Union represented, step based salary dependingon years of experience)

Qualifications
We'reseeking a dedicated clinical professional with:
Pharm D.degree and current CA Pharmacy license (required).
ASHP-accreditedresidency (preferred).
Completionof PGY1 Pharmacy Residency or equivalent 3-5 years experience in inpatienthospital pharmacy setting and/or in decentralized patient care pharmacy setting
Preferclinical background/residency in oncology and/or Board Certification inOncology.
Exceptionalcommunication, interpersonal, organizational and follow-through skills.
Knowledgeof and ability to perform age specific and disease specific pharmacokineticcalculations.
Ability tocompound intravenous admixtures and other sterile products.
