Schedule: Evenings, 4 days a week
Shift:75% (3 days) time in Emergency Room hours 1200-2200, other time spent staffing in Central pharmacy (1 day). Every 5th weekend staffing in the Central Pharmacy.

The role you’ll contribute:
The Clinical Pharmacist shall support the mission of the hospital and pharmacy department by preventing, identifying and resolving drug related problems to assure patient safety, improve patient satisfaction and minimize costs.

Actively performs all pharmacy functions and maintains active proficiency in:

·Dispensing and compounding systems

·Medication ordering systems
Provides primary coverage of direct patient care programs and multidisciplinary services
·Provides accurate, safe, timely, and appropriate medication therapy based on patient needs
·Places practice priority on the delivery of patient-centered care
·Identifies high-risk patients and implements interventions to improve quality and safety
·Formulates patient problem lists and accurate medication histories
·Monitors and tracks patients’ changing health care needs and implements, evaluates, and redesigns pharmaceutical care plans based on patient progress and evolving goals
Qualifications
The expertise and experiences you’ll need to succeed:
KNOWLEDGE AND SKILLS REQUIRED:
·Strong verbal and written communication, organizational, problemsolving, and customer service skills
·Flexibility and willingness to adapt to changes as necessary
·Works under minimal supervision, while using independent judgement
·Ability to interact constructively with supervisors, peers, and support personnel
·Ability to analyze data and follow up and resolve discrepancies
·Ability to keep accurate and complete records
·Ability to teach, precept, and facilitate experiential training
·Initiative to keep abreast of developments in the provision of pharmacy services
·Initiative to keep abreast of new information/products/developments in the field of pharmacy
·Proficiency in general hospital pharmacy operations and regulatory compliance
·Proficiency with hospital computer systems, pharmacy technology, and pharmacy automation
·Proficiency in hospital pharmacy practice, patient-centered care, advanced pharmacy practice, medication therapy management, and evidence-based medicine
·Working knowledge of the American Society of Health-System Pharmacists Accreditation Standards for Pharmacy Residency Programs
·Must be able to read, write and speak conversational English
·Basic computer skills and knowledge of Microsoft Office
·Performs other duties as assigned
EDUCATION AND EXPERIENCE REQUIRED:
·Bachelor of Pharmacy, Doctor of Pharmacy, or other advanced pharmacy degree
·PGY1 Pharmacy Residency or three years of clinical pharmacy experience
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
·Advance Cardiac Life Support (ACLS)
·Applicable State Pharmacy License
EDUCATION AND EXPERIENCE PREFERRED:
· Pharmacy ED Experience strongly preferred
LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:
·Board of Pharmaceutical Specialties certification
