SUMMARY: SUMMARY: The Director of Quality and Care Continuum is responsible for Care Continuum, Performance Improvement (PI), Clinical Documentation Improvement, and Payor Contracts. The primary responsibilities include leading the organization's continuum of care and clinical documentation improvement initiatives from a strategic and operation perspective by identifying care coordination and documentation gaps, developing strategies for implementation to close such gaps and working collaboratively with a multidisciplinary team to meet these goals.
