Schedule: PRN, as needed
Shift: Weekdays, with occasional weekend days.
Location: This position will perform patient home visits in the Palm Coast to Ormond Beach area and surrounding areas.

The role you’ll contribute:
Performs evaluation of the homecare patient to determine the type of speech or language disorder and the appropriate corrective therapy;provides rehabilitative services for swallowing, speech and language disorders; records activities and findings in the clinical record, submits reports to the physician regarding changes in the patient's condition or deviations from the plan of care; instructs other health team personnel and caregivers in methods of assisting the patient to improve and correct speech disabilities.
The value you’ll bring to the team:
Performs an evaluation and utilizes the findings to determine diagnosis (for SLP services), prognosis and plan of care in consultation with the physician.Determines specific interventions, duration and frequency needed to achieve anticipated goals and outcomes.
Observes and records activities and findings in the clinical record and reports to the physician the patient’s reaction to treatment and any changes in patient’s condition, or when there are deviations from the plan of care.
Instructs the patient and caregiver regarding the patient’s speech language improvement program including adaptive equipment and the home program.
Instructs other health team personnel including, when appropriate, home health aides and caregivers in certain phases of speech therapy with which they may work with the patient.
Selects, applies or modifies skilled intervention to the patient using various skilled interventions consistent with the state practice act to achieve the best outcome possible for the patient.Assesses patient response to interventions and performs reassessments as required.
The expertise and experiences you’ll need to succeed:
Minimum qualifications:
·Must be fluent in the English language
·Demonstrates moderate computer skills
·Master’s level graduate of a Speech Language Pathology program
·Current state of Florida Speech/Language Pathology license
·Current Florida driver’s license
·Basic Life Support certification
Preferred qualifications:
·Previous home health experience
·Knowledge of regulatory requirements
·Certification for Vital Stim
·At least 2 years of clinical experience as a speech language pathologist
·Experience working with adult and geriatric rehabilitation cases
·Home health experience within the past 3 years
·Current knowledge of homecare requirements and OASIS assessments
