About the Role:
Job Summary
Under the direction of the Registered Nurse, provides nursing care to assigned patient populations. Organizes patient care and sets priorities while assisting with the provision of patient care as established within the framework of the Georgia Board of Nursing Nurse Practice Act.
Minimum Job Qualifications
Licensure or other certifications: Current State LPN License.
Educational Requirements: High School Diploma or GED and LPN diploma and Licensure.
Minimum Experience: New graduate LPNs or those with less than one year of experience will be referred to the LPN residency program.

Demonstrates knowledge appropriate to care for pediatric, adolescent, adult, and geriatric patient populations specific to unit of practice.
Meets all technical certification requirements defined by unit (i.e. BLS, ACLS, PALS).
Demonstrates critical thinking skills.
Demonstrates sound clinical skills and judgment.
Demonstrates basic computer skills necessary to operate computer systems used on unit.
Essential Tasks and Responsibilities
Patient Assessment: The LPN assists with data collection and provides feedback to the Registered Nurse from a variety of sources to enhance clinical decisions about treatments and nursing interventions. Assists with the collection of data to reflect the patient's present condition and history with focus on immediate, interim, and long tern needs. Obtains data sensitive to cultural diversity, ethnicity, age, gender and lifestyle choices from multiple sources including the patient, significant other, healthcare providers, and medical records. Assists with the collection of pertinent data and reports findings to the Registered Nurse and other healthcare providers. Documents relevant data according to hospital standards.
Planning: The LPN provides feedback to the Registered Nurse in the development of nursing diagnoses and carries out the established plan of care and documents reassessments, interventions and patients' responses to interventions. Assists with discharge planning. Implementation: The LPN carries out therapeutic interventions established in the interdisciplinary plan of care. Implements interventions with safety, skill, and efficiency and adjust according to patient response. Documents patient response to interventions according to hospital standards. Teaches the patient and significant other regarding needs identified on admission and during the hospitalization. Documents teaching as established by hospital standards.
Evaluation: The LPN provides real time feedback to the Registered Nurse in the evaluation of interventions and treatments and how they affect patient health status and expected outcomes. Evaluates patient responses to nursing care and therapies. Documents responses to nursing interventions according to hospital standards. Involves the patient, significant other, and healthcare providers in the evaluation and revision of the plan of care.
Quality of Care: The LPN supports the evaluation of care activities, education, and practice environments to ensure the provision of safe, quality care. Uses quality improvement activities, research findings, and evidence-based practice to improve clinical care. Participates in quality improvement and assists to achieve unit-specific nurse-sensitive indicators and other indicators being monitored. Participates in activities that utilize results to initiate change in practice and to collaborate with other healthcare providers to institute improvements. Reviews charts for quality evaluation. Reports potential issues through the established chain of command. Delivers care that is non-judgmental, sensitive, and culturally competent. Develops a therapeutic relationship with patients that uphold humane/holistic health care congruent with the patient's needs, beliefs, and values. Adheres to established parameters for medication administration.
Education: The LPN participates in educational activities to improve clinical knowledge and enhance role performance. Completes annual competencies as indicated. Maintains records that provide evidence of competency and acquired knowledge, skills, and certification necessary for practice setting. Acquires a minimum of ten hours of continuing nursing education in addition to mandatory licensure requirements.
Collegiality: The LPN shares knowledge, skills, and observations with colleagues and others. Serves as a team member on assigned unit. Participates in the orientation of other employees. Provides constructive feedback regarding practice with the goal of facilitating professional growth. Contributes to a healthy and supportive work environment. Exhibits creativity, flexibility, and high personal standards for quality work. Collaborates with other members of the healthcare team to provide care.
