Overview
This role is a frontline population health worker who is also a trusted member of the community and/or has a close understanding of the community served. This trusting relationship enables the Community Health Worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competency of service delivery.
Responsibilities
Assist patients and families in navigating and accessing healthcare, community services, and resources
Promote adoption of healthy behaviors
Build and maintain relationships with patients via telephone, clinic sites, mobile health clinics, home visits, and community locations
Provide appropriate training and oversight
Support providers and Care Coordinators with care management and community outreach
Promote, maintain, and improve the health of patients and families
Behave professionally and promote values of respect, exceptional service, continuous improvement, safety, trust, and transparency
Pursue excellence in improving community health status

Assess social needs using a standardized social barrier assessment tool
Link participants with community resources to address health-related social needs

Work collaboratively with the multidisciplinary team to address medical, functional, and social barriers to care
Bridge gaps for previously un-engaged patients and those with lost contact
Identify, refer, and coordinate available community resources for non-medical needs
Ensure patient access to services
Register and schedule patients in the EMR
Accurately document in patients' electronic medical records (EMR), using Athena and/or Cerner
Document patient interactions for follow-up purposes
Provide patient and family education regarding follow-up needs during visits
Participate in organizational quality initiatives
Participate in multidisciplinary team meetings to optimize care delivery (e.g., Microsoft Teams virtual meetings)

Qualifications
Ability to travel up to 50% of the time for field-based work Schedule is Monday - Friday, 8:30am to 5:00pm with possibility of need-based flexibility
Required to have a dedicated work area established that is separated from other living areas and provides information privacy
Ability to keep all company sensitive documents secure (if applicable)

Minimum Qualifications High School Diploma or GED. At least two years related experience with community engagement and/or health (i.e., medical assistant, classroom assistant, CHW, outreach worker, volunteer, etc.). Additional education may substitute for required experience.
Preferred Qualifications:
3+ years of experience in working with at risk populations
2+ years residing within Sussex County
Previous experience as a CHW and knowledge of community resources highly desirable.
CHW Certification 1+ years of knowledge of Medicare/Medicaid population
Field based experience: Certified Nursing Assistant / Home Health Aide, or Medical Assistant
Bilingual fluency in Spanish and English
Experience working in managed care

Beebe Healthcare serves the Sussex County community with a medical center located in beautiful, historic Lewes, Delaware, and convenient outpatient locations throughout the county. Founded in 1916 by two physician brothers, Drs. James Beebe and Richard C. Beebe, the medical center in Lewes is a 210-licensed-bed, not-for-profit seaside community hospital. Beebe's specialized service lines include cardiac and vascular, surgical services, oncology, women's health, and orthopaedic services. Beebe offers an array of inpatient, outpatient, emergency, and diagnostic services.
Beebe's five satellite campuses offer imaging, physical rehabilitation, and lab services, as well as walk-in care centers. In addition, Beebe offers Wound Healing & Hyperbaric Medicine and Diabetes Management & Medical Nutrition Therapy.
With a mission to serve the community, Beebe also offers many free screenings and outreach programs through its Population Health Department; and advanced medical care for those who are recuperating at home through Beebe Home Care Services.
Beebe is also training the next generation of nurses through the Margaret H. Rollins School of Nursing, located behind the Medical Cente...r in Lewes.
Beebe's Medical Staff has grown tremendously since the early 1990s. Employed physicians through Beebe Medical Group and our affiliated physicians on Beebe's Medical Staff now serve patients from Milford to Millville in specialties including cardiac surgery, dermatology, endocrinology, family practice, gastroenterology, general surgery, internal medicine, pediatric neurology, and surgical oncology.
