This position is for Advanced Primary Care Clinics where you will care for patients with medical and social complexities through value based care. You will build relationships with your patients, focus on preventive care and chronic conditions, and coordinate patient care needs as part of the interdisciplinary Advanced Primary Care team. In addition to rooming and managing clinic flow for the provider, you will provide immunizations and medications, perform procedures like wound care, POCT and lab draws, as well as assist providers with procedures. This role offers you a great opportunity to apply your motivation, organization, substantial clinical competency, self-direction, interpretation of information and judgement skills and abilities to achieve the organizations goal of providing safe, reliable, and effective heath care excellence, patient-centered care and community engagement.
Essential Functions
Independently triages patients in person, via telephone and secure messages, utilizing protocols, guidelines, and standards.
Provide patient-centered education of disease processes and wellness opportunities
Use assessment and analysis skills in delivering medical regimens as directed by physicians and APPs.
Assist patients and families toward self-management of care including goal setting for management of chronic disease, improving medication adherence, creating a plan for behavioral change and coordinating community resources via nurse visit, telephone management, or other contacts, as needed
Delegation of selected patient care functions to others in accordance with their education, credentials, and demonstrated competence.
Serve as resource for staff, colleagues, physicians and advanced practice providers to positively impact CMS core measures and National Patient Safety Goals.
Use critical thinking skills in making decisions regarding patient care, which includes knowledge and integration of relevant standards, resources, and data.
Advocate for patients and families, ensuring sensitivity to cultural and ethnic practices and beliefs/for ethical and holistic care promoting the autonomy, dignity, and health of patients from diverse ethnic and social backgrounds.
Qualifications
Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service or internal candidates hired before January 1, 2021 may be considered in satisfaction of this requirement in lieu of an education agreement
Masters degree in nursing preferred
Current Registered Nurse licenses in Washington State or Multistate license endorsement
Current BLS Certification
Three (3) years of recent clinical hospital/ambulatory experience
Knowledge of care continuum (inpatient, ambulatory, post-acute, etc.) preferred
Certification as Certified Case Manager (CCMC), Accredited Case Manager (ACMA), Care Coordination and Transition Management (AAACN), Ambulatory Care Nursing (AAACN), or similar required or obtained within 3 years of hire (Employees hired prior to 8/1/2023 will have this requirement waived)

Pay and Benefit Expectations
We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $96,689.00 - $161,160.00 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicants years of experience align.
