The community you’ll be caring for:
• Winter Park Memorial Hospital has continuously served the residents of Winter Park and its surrounding communities for more than 50 years
• Chartered in 1951, the hospital has grown from the visionary efforts of a handful of Winter Park residents and community leaders, to a 307-bed acute care facility that is a model of community health and wellness
• Over the years the hospital has continually expanded to meet the needs of the community, adding an upscale obstetrics and Level II Neonatal Intensive Care Unit at The Dr. P. Phillips Baby Place, cancer care at the AdventHealth Cancer Institute, Winter Park, and state-of-the-art surgery, recovery and rehabilitation at the AdventHealth Orthopedic Institute
The role you’ll contribute:
- Coordinates appropriate social work and case management services to patients and their immediate families.
- Serves as the client advocate and assists in the coordination of the rehabilitation process from admission to discharge.
- Adheres to AdventHealth Corporate Compliance Plan and to all rules and regulations of all applicable local, state and federal agencies and accrediting bodies.
- Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
The value you’ll bring to the team:
- Evaluates, re-evaluates, establishes, implements and adapts an individualized discharge plan of care according to the psychological, social, functional, and financial impact of illness on patient/family according to standards of practice, payer source and patient’s needs.
- Appropriately serves as the liaison to the payer source obtaining authorization and updating as needed. Promotes efficient utilization of healthcare services at the most appropriate level of care as determined by professional standards.
- Establishes and maintains methods of communications with referring and supporting healthcare individuals and organizations to ensure clear and concise exchange of necessary information in a timely manner. Facilitates patient’s movement through the program by providing proactive planning and creating solutions to facilitate seamless care. Advocates for patients’ ongoing and discharge needs.
- Coordinates contact and refers to appropriate agencies, services and resources to set up for patients’ post-discharge needs including durable medical equipment and follow-up care.
- Documents accurately, completely and timely with legible handwriting. Uses measurable and objective terminology, following standards of practice, insurance guidelines and department/hospital policy and procedures.
What you will need to succeed:
EDUCATION AND EXPERIENCE REQUIRED:
- Bachelor’s degree in Rehabilitation or Social Services or RN
EDUCATION AND EXPERIENCE PREFERRED:
- MSW
- Experience in Rehabilitation
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
- If RN, current valid State of Florida or multi state license as a Registered Nurse
- Basic Life Support (BLS) certification
