SUMMARY OF POSITION:
The System Formulary Clinical Coordinator is responsible for the development, implementation, and management of the formulary system within the pharmacy platform. This role ensures that the system formulary is evidence-based, cost-effective, and aligns with patient care standards. The coordinator works closely with the System Pharmacy and Therapeutics (P&T) Committee, physicians, and other healthcare professionals across Penn State Health (PSH) to manage medication utilization, optimize therapeutic outcomes, and control drug costs.
NOTE: This role will manage the formulary for all PSH entities.
MINIMUM QUALIFICATION(S):
Doctor of Pharmacy or Bachelors Degree in Pharmacy required.
ASHP accredited PGY 1 Pharmacy residency or Three (3) years clinical experience required.
Minimum Three (3) years of experience in a clinical pharmacy practice required
Pharmacist License - Current Pennsylvania Licensure or eligibility required.
PREFERRED QUALIFICATION(S):
ASHP accredited PGY 2 Pharmacy residency or Six (6) years clinical experience are preferred.
