Hepatology Nurse Navigator
Responsible for serving as the first reliable point of contact for Patients, Families and Providers.Principal Accountabilities:1. Serves as patient and family advocate for liver patients. 2. Coordinates the scheduling of appointments to expedite the plan of care and to promote continuity of care. 3.
