The Medication Access Specialist assists in finding access to affordable medications for the insured and uninsured ACO patient population.

Remote
Triage medication cost referrals for any available assistance programs
Assist with Medication-related Quality Measure Improvement
Coordinate patient care needs in collaboration with and under supervision of ACO Clinical Pharmacist Specialists
Provide medication access related education to ACO and BJCMG as applicable
Pharmacy Tech experience preferred

Role Purpose
Serves as the primary resource between the care management team, Providers and other patient care entities on projects that impact the care management of patients in the ACO (Accountable Care Organization). Responsible for collecting, developing and monitoring project documentation to implement and sustain patient-centered care activities.
Responsibilities
Collaborates with directors, managers and other disciplines to plan and implement projects related to patient care area.
Assembles project plans and teamwork assignments, directing and monitoring work efforts on a daily basis, identifying resource needs, performing quality review, and escalating functional, quality, timeline issues appropriately.
Ensures overall coordination, status reporting and stability of projects or operations-oriented work efforts.
Establishes methods to monitor the availability of reportable data and assists with planning and implementing new projects.
Works directly with leadership on internal business development activities, integration of business and quality goals and objectives, scope of services, and compliance planning.
