Job Summary
Collaborates with physicians, inpatient nursing units, and NCHS leaders to facilitate the patient admission, transfer, and discharge (ADT) processes. Rounds the nursing units, monitors and identifies bed availability to support patient throughput, and effectively utilizes inpatient beds. Attends bed control meetings and collects, monitors, and analyzes data related to bed availability, daily census, and the bed control process. Under limited direction, provides an effective admission and discharge process for patients and supportive clinical resources, to support patient care units, as related to patient admission and discharge. Supports and upholds quality standards of a Magnet designated hospital and Patient Bill of Rights and practices under the supervision of the Nursing Manager/Director.
Qualifications
Minimum Job Requirements
- RN Licensure within the State of Florida or Multi-State Enhanced Nursing License Compact (eNLC) - maintain active and in good standing throughout employment
- American Heart Association BLS - maintain active and in good standing throughout employment
- American Heart Association PALS - maintain active and in good standing throughout employment
- A minimum of two years RN experience in a pediatric healthcare environment
Knowledge, Skills, and Abilities
- BSN Degree or currently enrolled in BSN program.
- 1 year of ADT RN experience preferred.
- 2 years of experience as an RN in ICU environment preferred.
- Nursing experience in a pediatric healthcare ICU setting strongly preferred.
- Bed Control experience preferred.
- ACLS & NRP preferred.
- Strong problem solving, critical thinking, and conflict resolution skills.
- Ability to manage multiple priorities and strong time management skills.
- Effective verbal and written communication skills.
- Possess proficiency in computers and Electronic Health Record (EHR) preferred.
- Career Development: National certified preferably within 5 years of hire.
- Works effectively in a collaborative environment.
- Ability to apply principles of evidenced-based practice, analytical thinking, and service excellence in practice.
- Able to analyze and interpret data and utilize the information to make judgments regarding patient care.
- Demonstrates a complete understanding of the EMTALA (Emergency Medical Treatment and Active Labor Act).

Job Specific Duties
- Collaborates with physicians and healthcare institutions to facilitate the patient admission and transfer processes in accordance with NCHS policies/procedures for timely bed placement.
- Performs initial patient assessment, initiates care as appropriate, reconciles medications, obtains/processes physician orders, and completes documentation with patient and family.
- Utilizes advanced assessment skills to complete admission documentation to include; assessment/admission history, medication reconciliation, core measures, anticipated discharge needs, etc.
- Completes all nursing multi-system assessment that includes head to toe, vitals, dressing changes, suction, feeding, bathing ,med administration, and repositioning.
- Completes the emergency and direct admission process and assists with the patient transfer process to designated nursing unit.
- Assists with transporting admitted patient to inpatient unit and effectively completes the nursing hand-off process. Also effectively completes the hand-off process at shift change.
- Works closely with department leadership to ensure acceptance of patients is in line with the admission and exclusionary criteria of the hospital and is appropriate for admission.
- Participates in bed control meetings; provides updates on patient bed placement and ADT processes. Compiles, reconciles, and distributes bed status reports to designated NCHS staff.
- Identifies and recommends quality and process improvement initiatives and assists with the development and execution of action plans.
- Coordinates the transfer of patients from other hospitals within the United States and outside the United States with the Transport Team.
- Rounds inpatient units to identify and evaluate inpatient bed availability, facilitates effective utilization of inpatient beds, facilitates admissions process, and drives throughput.
- Acts as the bedside nurse to assist physician with medical assessments, and utilizes nursing skills to perform tests such as Flu, RSV, and Strep.
- Accesses electronic health record independently according to hospital policy & completes accurate and timely documentation in the patient's electronic health record.
- Promotes a safe 'error free' working environment by consistently performing thorough hand-offs and infection prevention measures.
- Assesses all body systems & ensures safe, effective/efficient care delivery with emphasis on absence of medication errors, falls avoidance, ouch-free procedural pain, and pain reduction.
- Accurately completes the discharge planning process per policies and procedures. Communicates what aspects of the discharge process has been completed with the RN responsible for the patient.
- Completes the Admission Data Base, Allergy Assessment, Home Medication Assessment, and Health History on scheduled, unscheduled or direct patient admissions on patient care units.
- Supports the patient discharge process in terms of medication reconciliation and finalizing discharge orders; makes follow-up appointments, calls in prescriptions, completes discharge summary, etc.
- Facilitates throughput by identifying pending & confirmed discharges; communicates with department facilitator and Ops Administrator for direction of areas with priority for admission & discharge.
- Collaborates with nurses and providers to ensure accurate discharge medications and patient education. Assists Nursing staff in completing other discharge tasks.
- Reviews and discusses the discharge plan with the patient, family, and professional staff to ensure everyone understands and agrees to follow it.
- Ensures coordination of discharge services with other agencies, obtaining medications and supplies, or arranging for professional/clinical services occur.
- Monitors discharge process statistics and metrics on weekly basis with Manager of Nursing Operations and Director of Operations.
