The Medical Instrument Technologist (MIT), Ultrasound (US), performs general diagnostic ultrasound studies in radiology at an outpatient clinic in Chattanooga. The purpose of these exams is to evaluate and diagnose patients with suspected pathology and provide continuity of follow-up to requesting providers to carry out treatment plans. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Medical Instrument Technicians must be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a, this part. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the core competencies defined at the GS-8 full performance level, positions at this grade may be lead, supervisory, or non-supervisory with specialized advanced knowledge and assignments. At least 1 year of experience comparable to the next lower grade level, which is directly related to the duties of the position to be filled, and must fully meet the KSAs at that level. This would be specialized experience which demonstrates possession of the knowledge required in order to provide all aspects of diagnostic ultrasonography services. In addition, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs designated by an asterisk (*): Demonstrated Knowledge, Skills, and Abilities. KSAs 1, 2, and 3 are appropriate for all positions at this level. KSAs 4 and 5 are to be used in accordance with their appropriateness for the specific assignment. 1. Knowledge of the more complex examination and treatment procedures and techniques in order to provide training and supervision in these procedures, to evaluate and initiate performance improvement projects, and to write policies, procedures, and protocols that pertain to diagnostic ultrasonography. *2. Knowledge of JCAHO and other regulatory requirements. 3. Ability to communicate effectively and professionally with employees at varying grade levels. *4. Ability to manage and supervise employees. *5. Ability to provide, or provide for, staff development and training. Physical Requirements: The physical demands of the work require extended periods of sitting, moderate walking, and standing. Work involves constant bending, lifting, twisting, and carrying moderately heavy items, or moving equipment around, such as the ultrasound machine. The work requires moderate lifting, usually less than 50 lbs. The work requires pushing wheelchair and stretcher patients. Positioning patients who are disabled on the stretcher requires considerable effort and working in awkward positions. Lifting of totally incapacitated patients is done only with the help of lifts and/or other employees. The technologist must be competent in the production, use, recognition, and analysis of ultrasound images and patterns used for patient diagnosis and treatment. The sonographer is regarded as the expert source and the interpreting radiologist relies heavily on the skills, knowledge, and abilities of the sonographers in providing a final interpretation. The technologist requires knowledge of image acquisition in ultrasound and Picture Archiving and Communication Systems (PACS) and is able to carry out his/her assignment independently. Duties include but not limited to: Performs ultrasound examinations ranging from the routine to specialized in an outpatient setting. Receives and identifies patients using two (2) patient identifiers, explains methods of procedures, assists movement of patients to and from wheelchair, table, positions the patient and performs examinations as requested. Provides care to the patients to allay anxieties, answer questions, familiarizing them with the procedures and equipment to be used. The technologist interprets the requests or instructions from the radiologist or clinician. Receives assignments for a variety of difficult examinations and works independently. Follows departmental Policy and Procedure Manual. Must be able to handle possible patient reactions using CPR (i.e. Basic Cardiac Life Support (BCLS) Certification is required. Knowledge of anatomy, cross sectional anatomy, and physiology such as the location, appearance and function of the various major and minor systems applicable to sonographic illustration in order to interpret the requested examination accurately. Thoroughly comprehends all processes for reusable medical equipment (RME) including initial training, proper setup, use and reprocessing (cleaning, disinfection, sterilization, and preparation of equipment to full readiness for its subsequent use) and maintenance according to local standard operating procedures. Cleans and maintains all equipment in accordance with Infection Control policies, manufacturer guidelines, biomedical guidelines, and Joint Commission criteria. Reports any unsanitary conditions or equipment malfunctions timely to the supervisor. Team member works independently, under the direction of the Chief Technologist. Technologist confers with radiologists and/or supervisor on more difficult problems. The team member must follow the chain of command and resolve issues at the lowest level possible. Provides care and/or services appropriate to the age of the patients being served, and which is appropriate to the cognitive; emotional, and chronological maturation of the patient. Protects printed and electronic files containing sensitive data in accordance with the provisions of the Privacy Act of 1974 and other applicable laws, Federal regulations, VA statutes and policy and VHA policy.
