To perform this job, an individual must perform each essential function satisfactorily with or without reasonable accommodation.
·Interacts professionally with residents/families; treats residents and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families.
·Effectively and appropriately communicate with residents, physicians, family members and co-workers, demonstrating customer service excellence through application of positive language principles, appropriate response and use of constructive problem-solving techniques which foster positive outcomes.
·Maintains accurate and continued documentation appropriately for all assessments, treatments, and evaluations performed. Documentation meets current standards and policies.
·Ability to, under direct supervision of an RN, perform an appropriate assessment on all residents as related to the care requested and provided and reassessments as per policy.
·Follows the six (6) medication rights and reduces the potential for medication errors.
·Demonstrates knowledge of medications and their correct administration based on resident?s clinical condition.
·Assist residents with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
·Implement a plan of care which is appropriate to the resident based on assessments and goals established by the interdisciplinary care team.
·Ability to perform plan of care interventions, including specimen collection and wound care.
·Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the resident and family regarding pain management as needed.
·Ability to communicate with physician, any changes in resident?s condition and follow through until appropriate action is taken. Note and follow physician orders in timely manner.
·Ability to perform waived testing (point-of-care testing) per Clinical Laboratory?s and the resident care unit?s policies and procedures. (See your state?s scope of practice for LPNs/LVNs.)
·Give a thorough report to oncoming shift by participating in briefs and by documenting appropriately on the 24-hour report.
·Supervise the Certified Nursing Assistants working on shift as needed to accomplish the plan of care for each resident.
·Performs other reasonably related job duties as assigned or requested, within scope of practice.
Qualifications
Education:
Licensed Practical Nursing Degree, Required
Required Licenses:
Must have current Mississippi LPN (Licensed Practical Nurse) licensure or Compact State.
Required Certifications:
Basic Life Support (BLS) Certification required within 90 days of hire or transfer.
Required Skills:
·Requirements will include successful completion of orientation checklist and skills/credentials/competency requirements of specific area of work.
·Able to maintain a high awareness of patient satisfaction and teamwork.
·Attention to detail.
·Ability to respond to and cope with emergency situations professionally and calmly.
·Adaptable to performing well under pressure in a stressful environment and taking appropriate action.
·Must be flexible, able to work as needed for appropriate departmental coverage.
·Basic computer knowledge; become proficient in Electronic Medical Records (EMR) systems.
·Requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision.
·Demonstrates proficient time management skills.
·Sufficient proficiency in speaking, reading, and writing the English language necessary to perform the essential functions of this job, especially with regard to activities impacting patient or employee safety or security. Additional languages preferred.
