Summary:
The Licensed Vocational Nurse (LVN), under the direct supervision of a Registered Nurse, administers patient care. The LVN will complement the nursing process assisting in the individualized care of the patient in an attempt to assist the patient in adapting to the disease process and achieve the highest level of wellness.
Responsibilities:
Maintains consistency with Administrative and Departmental policies with appropriate behavior, dress, attitude, attendance, confidentiality, professionalism, and reliability.
Insures professional discretion and confidentiality of hospital related information concerning patients, physicians, associates and visitors.
Complies with conditions of employment as stated in the associate handbook and Administrative and Departmental policies.
Complies with dress code policy.
Limits non-emergency personal telephone calls to breaks and meal times.
Assists the RN in the nursing process to provide information for development of the plan of care.
Supports the RN by providing information for the development of the plan of care.
Promptly reports identified changes in the patient's condition.
Reviews and utilizes diagnostic data, medical orders and nursing assessment in order to provide continuity of care in accordance with the plan of care.
Demonstrates an ability to respond appropriately and/or report abnormal clinical/laboratory findings or results.
Sets priorities for care of patients based on acuity and/or patient/family preference.
Accurately reports and documents the patient's symptoms, responses, and status to reflect the nursing care plan developed by the RN.
Safely performs and documents nursing procedures required by patient assignment consistent with scientific principles of nursing and policy and procedure.
Promotes and participates in patient/family education and counseling based on health needs.
Reports learning needs of the patient and family to be included in the teaching plan of care.
Documentation reflects teaching, explains tests, treatments, medications, procedures, and disease process to the patient and family as needed.
Assists in ensuring that discharge needs are met including food and drug interactions, test, procedures, medications, and equipment and return appointments.
Demonstrates competence to perform skills for which training has been provided, including:
Completion of required CBSA.
Unit specific skills.
Demonstration/completion of annual competency.
Participates in nursing research and/or quality improvement activities.
Participates in unit based quality improvement programs/meetings.
Participates in data collection for unit based quality improvement programs.
Requirements:
Education/Skills
High school diploma or equivalent.
Basic Computer Knowledge
Ability to communicate effectively, both verbally and written.
Successful completion of LVN / LPN training.
Experience
Prefer 6 months of clinical practice in an acute care environment.
Licenses, Registrations, or Certifications
Maintain current state licensure and provide the hospital with verification.
Maintain current BCLS certification.
