Occ Summary
Level IProvide psychosocial assessments, diag nosis, and treatment,as well as discharge planning to and consultation a bout patients andfamilies to assist them and the health care team in cop ingwithpatient's hospitalization, illness, diagnosis, treatment, and/or lifesituation, including emotional, mental, and substance abusedisorders .Level IIFunction in a supervisory role as a teamleader in addition to t heduties and responsibilities of Level I.
Work Performed
Level IProvide psychosocial assessments of patients and familiesto identify emotional, social, and environmental strengths and problemsrela ted to their diagnosis, illness, treatment, and/or lifesituation.Formula te, develop, and implement a comprehensivepsychosocial treatment plan ut ilizing appropriate clinical social worktreatments and interventions. In terventions may include crisisintervention, brief and long-term individu al, marital, family and/orgroup therapies as well as grief and bereaveme nt work.Screen,identify, diagnose using DSM nomenclature, treat and mana ge mentalhealth and/or substance abuse problems in patients and family m embers.Provide training to other health care professionals in same. Mayp erform these functions independently or as part of a team.Assist with sc reening, identification, diagnosis, managementand treatment of victims o fabuse, neglect, domestic violence, rape,etc. Provide training to otherh ealth care professionals insame.May participate in on-call or after hour scoverage.Provide consultative services to health care teammembers withi n scope of care definitions as needed.Maintain aworking knowledge of rel evant medical/legal issues that impact onpatient care, e.g., advance dir ectives, child and elderabuse.Provide education to patients and families around issuesrelated to adaptation to the patient's diagnosis, illness, treatmentand/or life situation.Participate in multi-disciplinary health care teams and provide leadership in representing clinical social workpe rspective/liaison with patient and family.Provide teachingand trainingon tribute to the academic mission of themedical center and health systemfo r students, staff, and faculty from social work andother disciplines.Cby participating in academicactivities such as committees, conferences,pub lications, teaching andresearch.Maintain working knowledge of and liaiso n withcommunity agencies and resources.Arrange, procure, andcoordinate p atient/family pre and post hospital needs.Knowledgeof hospital, medicalcenter and/or health system resources to accessand provide for patient c are needs.Document assessment, plan,interactions, and interventionsaccor ding to departmental, hospitaland/or health system guidelines and standa rds.Maintain recordsand statistics in accordance with department, hospit al, medical centerand/or health system policies.Attend and participate i n staff,committee, department, and other administrativemeetings.Particip ate in department, hospital, medical center,and/or health system continu ous quality improvementefforts.Participate in regular supervisory confer ences; keepcurrent with social work and health care developments and see k toincrease further enhancement of job related knowledge.LevelII Perfor m all of the duties and responsibilities of LevelI.Provide clinicaland a dministrative supervision for socialworkers and clinical social workers on the team.Lead teammeetings and manage coverage.Participate in hiring andperformance management processes.
Knowledge, Skillsand Abilities
Level I Adherence to the practice standards andethical gu idelines of NASW and the NC Certification Board for SocialWork. Excellen t verbal and written communication skills. Asolid knowledge of clinical social work practice. Excellentassessment, interviewing, and counseling skills with expertise inmental health and substance abuse areas;familiar with psychopathologyand with range of therapeutic interventionsincludin gpsychopharmocotherapy; and if working independently, must knowwhen tore fer for further medical assessment. Ability to work effectivelyand auton omously in a self directed role. Highly collaborative withstrong interpe rsonal and team building skills. Ability toprioritize multiplework assig nments and manage time efficiently.Flexibility in accepting diverse work assignments and managing stressrelated to change. Basic computer skills Level II Sameas Level I plus strong supervisory and leadership skills. A bilityto resolve/manage conflict. Ability to help implement departmental ,hospital and/or DUHS initiatives.
