PIH Health is seeking a compassionate and dedicated Nurse Practitioner (NP) to join our collaborative Hospice care team.
As a Hospice NP, you will be a key member of a multidisciplinary group of healthcare professionals working together to deliver compassionate, patient-centered care to individuals at the end of life. You will help manage symptoms, provide comfort, and offer emotional and practical support to both patients and their families during this meaningful and important phase of life.

Requirements:
Current California Nurse Practitioner License
Master’s degree in nursing from an accredited nurse practitioner program
Board Certification: Board certified as a Nurse Practitioner
At least two (2) years previous experience clinical practice in hospice, palliative care, chronic care management
Achieve and maintain hospital medical staff membership and clinical privileges.
Evidence of continuing education and self-development to support license requirements
Current Basic Life Support Card
ACLS certification required
Willingness and ability to use personal auto to drive to meetings, patient visits, and special events or to carry out assignments as requested. This will require proof of California driver's license and proof of insurance.
Preferred:
Management skills, expertise and availability to manage patients in different post-acute environments.
Current Board Certification in Hospice and Palliative Medicine is highly desirable.
At least five (5) years of clinical practice in hospice, palliative care, chronic care management
