POSITION OVERVIEW: Delivers high quality and compassionate care to the Rehabilitation patients by managing the physical and psychosocial care needs of patients and their families. Performs assessments of patients to address common medical and rehabilitation issues across a wide variety of diagnoses including stroke, spinal cord injury, brain injury, trauma patients, etc. As needed, assist with coordinating care with consulting services for patients on the inpatient rehab unit.
Works in collaboration with other Physicians and multi-disciplinary inpatient rehab staff. Primary responsibilities include rounding on the patients on the inpatient rehab unit to address medical and rehabilitation issues, ensure compliance with Medicare rules, coordinate discharge and post-hospital care needs for successful transition to the community. Additional responsibility to assist with consult service for initial and follow-up recommendations and coordinating with admissions team. Day-to-day patient contact/rounding/care management will actively encourage and include professional input from members of the interdisciplinary team. Accurate and timely completion of all clinical and professional records and documentation to assure compliance with local, state and federal regulatory, licensure and accreditation requirements.

CALL EXPECTATION: 1 weekend a month
ESSENTIAL FUNCTIONS AND DUTIES:
Clinical Duties:
Perform comprehensive initial assessments to determine rehabilitation needs.
Develop individualized treatment plans focusing on functional recovery.
Conduct regular rounding to monitor progress, address complications, and adjust care plans.
Oversee medical management, including pain control, spasticity treatment, and addressing co-existing conditions (e.g., diabetes, hypertension).
Coordinate medications and therapies to maximize recovery while minimizing side effects.
Lead team conferences with therapist, nurses, case managers, and other specialists to ensure coordinated care.
Provide guidance to physical, occupational, and speech therapist regarding therapy goals and medical considerations.
Collaborate with rehab psychologists to address cognitive, emotional, and behavioral challenges.
Work closely with nursing staff to ensure appropriate patient monitoring and care delivery.
Provide education on diagnosis, prognosis, and rehabilitation expectations.
Counsel patients and families on adaptive strategies, mobility aids, and community reintegration.
Discuss long-term care planning, including outpatient rehab, home modifications, and caregiver training.
Determine readiness for discharge and coordinate appropriate follow-up care.
Work with case managers and social workers to arrange outpatient therapy, home health services, or post-acute care.
Communicate with primary care physicians and specialists to ensure continuity of care.
Provide Outpatient follow-up for patients discharged from IPR.

Complies with all applicable laws, regulations, policies and procedures, supporting the health systems corporate integrity efforts by acting in an ethical and appropriate manner, reporting known or suspected violations of applicable rules, and cooperating fully with all health systems investigations and proceedings.
Keep all licenses and appropriate certifications current and unrestricted to maintain privileges at a St. Charles Health System hospital sufficient to permit performance of job duties.
Increases understanding and collaboration throughout the medical community.
Maintain compliance of hospitals Medical Staff Bylaws, Rules and Regulations, as required by SCHS Board of Directors.
EDUCATION:
Required:
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
Residency completion in Physical Medicine & Rehabilitation.
Board Certification by American Board of Physical Medicine and Rehabilitation (ABPMR) eligible. Expectation to be certified within 2 years of employment.
Complete annual fire/safety, etc. education in-services as required.
Attend mandatory educational offerings as described in SCHS policies/procedures and/or as required by other regulatory bodies.
Complete annual CME as required to maintain license and appropriate Board certification.
Complete in a timely manner assigned Computer Based Learning modules.
Attend mandatory medical staff meetings quarterly, and staff meetings (TBD)

LICENSURE/CERTIFICATION/REGISTRATION
Required:
Unrestricted Physiatrist Medical License in the state of Oregon.
Unrestricted Federal DEA number.
Unrestricted ability to participate in Medicare / Medicaid, or other federal or state governmental health care programs.
Valid drivers license and the ability to meet SCHS driving requirements.
Current BLS certification required.
Additional certifications may be required based on privileges granted to provider by Medical Staff Services at SCHS Inpatient locations.

EXPERIENCE
Required:
Experience using EMR (Electronic Medical Records) software.
Advanced written and verbal communication skills to effectively interact with a diverse population and professionally represent St. Charles Health System.
Experience working in a team environment and proven ability to collaborate, manage facilitation and consensus building among health care professionals and agencies to achieve expected results.
Preferred:
Previous experience as an Inpatient Rehabilitation Physiatrist.
Spinal cord injury or Brain injury fellowship.
