Summary

The Telehealth Clinical LVN (TC-LVN) serves in a clinical role for Telehealth Store and Forward programs including tele retinal imaging - tele dermatology imaging - clinical video telehealth - and Virtual Care Manager. The TC-LVN trains employees to use tele health virtual modalities to support clinical care.

Qualifications

Basic Requirements:

• Citizenship

• Proficiency in written and spoken English
• Licensure
• Full - active - current and unrestricted licensure as a licensed practical or vocational nurse in a State - Territory or Commonwealth (i.e. - Puerto Rico) of the United States - or District of Columbia
• An LPN/LVN who has or has ever had - any license(s) revoked - suspended - denied - restricted - limited - or issued/placed in a probationary status may be appointed only in accordance with the provisions in chapter 3 - section B - paragraph 16 of this part
• Education
• Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant

• Exceptions include: Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment
• VHA employees who were converted to appointments under 38 U.S.C. 7401(3) implementation of Public Law 98-160 - enacted November 21 - 1983 - and for VHA employees converted to appointments under 38 U.S.C. 7405 in implementation of Public Law 99-576 - enacted October 28 -1986

• Individuals granted a license by a jurisdiction that does not require graduation from an approved school
• Grade Determinations: GS-6 Experience: Completion of at least one (1) year of additional qualifying experience at the GS-5 level or equivalent - fully meeting all performance requirements for the GS-5 LPN/LVN
• LPN must be technically proficient in initiating - performing and completing assigned duties in providing car to variable patient populations
• Possess knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions

Duties

• The TC-LVN trains Veterans for successful virtual clinical appointments with providers by providing video test calls prior to scheduled appointment or setting the patient up in clinic for a virtual meeting with a provider off-site
• Duties include: Plans - coordinates - implements - manages - and evaluates clinical applications of telehealth within and between the SCI/D Center and SCI/D spoke sites as well as other VA and non-VA sites of care
• Collaborates with staff at other SCI/D Centers - SCI/D spokes - and other VA and non-VA points of care to coordinate the establishment and maintenance of SCI/D telehealth and other virtual care programs
• Ensuring that SCI/D telehealth operations - goals and priorities are complementary and congruent with SCI/D Center and spoke operations and with facility - VISN - and national VHA strategies
• Provides expertise to motivate SCI/D clinical teams to adopt and integrate telehealth and other virtual care technologies into their delivery of patient care
• This will include the conception - articulation - and development of new telehealth program initiatives and the maintenance of existing programs
• Serves as the on-site expert on telehealth-related policies - directives - regulations - and requirements and acts as a resource in planning - reviewing - or revising policies and procedures that impact clinical provider training and patient care delivery using telehealth modalities
• Maintains knowledge of current telehealth practices and updates to telehealth-related technologies - policies - directives - regulations - and requirements through attendance of relevant trainings - conference calls - in-person conferences - communication with colleagues and ensures that this knowledge is conveyed to others who will benefit from it
• Coordinates activities geared towards maintaining and improving existing telehealth programs through the establishment of program objectives - standards of care and standards of practice which address the specific clinical needs of Veterans with SCI/D
• Oversees the management of telehealth resources and processes to ensure that effectively utilized Provides direct patient care in accordance with national telehealth store and forward program recommendations and guidelines
• Serves as an expert resource and sets up - adjusts - and maintains specialized clinical equipment - computers - tablets - cameras - and other equipment with multiple features and precision metrics
• Plans - coordinates - manages - and evaluates care coordination of patients with PACT teams to incorporate telehealth imaging and video technologies
• Performs \"Look Ahead\" for telehealth clinic schedules and prepares for upcoming appointments by coordinating pre-appointment labs - testing - and patient self- monitoring records
• Communicates with HAS - PACT - Specialty Clinics - and other VAs to efficiently provide telehealth services
• Performs video test calls with patients in preparation for upcoming video visits with providers
• Participates in clinic readiness ensuring maintenance of imaging equipment and ordering supplies
• Provides telehealth video/virtual visit training for clinical staff
• Provides patient education in downloading - troubleshooting - and effective utilization of virtual health tools and guides others to perform this education
• Serves as expert resource to staff and patients on new and existing telehealth applications including MHV - VA Video Connect - Annie Health Subscriptions - and Mental Health Applications
• Supports telehealth clinical operations - training - and cross coverage
• Format - interpret and distribute data to stakeholders on a routine and as needed basis
• Maintains telehealth's dashboard to display priorities and trended data
• Supports and assists with managing the Tele-Observation Program at the VA San Diego
• Cross trained to cover urology - inpatient and outpatient clinic & tele-observer program
• Assists with completing Binax testing during COVID-19 Outbreaks
