Summary:

An RN Transfer Access Specialist is a critical part of the Access and Navigation Center by accepting patients for Transport via ???Quick Accept Guidelines??? (QAG), while managing bed assignments and mobilization of the Transport Team.

Responsibilities:

1. • Responsible for promoting and maintaining quality patient care through effective management of the activities that come through the Access and Navigation Center.
   • Accepts patients for Transport via ???Quick Accept Guidelines??? (QAG) escalating to ED/Specialty Provider when required.
   • Triage calls to the appropriate Specialty Medical Care Provider/team.
   • Manage EPIC Grand Central Bed Assignment Process. Acts as primary point of contact for patient bed and unit assignment, based on urgency of care, policy guidelines and departmental capacities.
   • Communicates with appropriate leaders when situations may have serious impact to patients, staff or the hospital.
   • Participates in periodic review of the Access and Navigation Center operations, policy and procedure updates, continuous improvement initiatives.
   • Maintains operational aspects of the unit in collaboration with Support Services.
   • Other duties as required.
