You will create and implement exercise programs for inpatient cardiac population initiating cardiac rehab in decompensated heart failure patients. Additionally, promoting and referring outpatient cardiac rehabilitation through collaboration with Nurse Practitioners and Attending Cardiologists. And you will serve as unit fall liaison by attending monthly meetings and keeping up with quarterly training.

Nursing Divisions 9100/9200 are 25 bed high risk cardiology units where you will find many exciting challenges and opportunities for learning. Many of the patients that you will care for require advanced medication infusions and post cardiac procedure care. Our team consists of registered nurses, patient/student nurse technicians, and house staff of residents, interns and other medical professionals.

Role Purpose
Creates and implements exercise programs for inpatient cardiac population initiating cardiac rehab in decompensated heart failure patients. Promotes and refers outpatient cardiac rehabilitation through collaboration with Nurse Practitioners and Attending Cardiologists. Serves as unit fall liaison.
Responsibilities
Ensures safe and appropriate exercise programs through design, implementation and management of the exercise programs to meet client needs
Provides and documents clinical exercise testing, assessment, and exercise prescriptions and exercise training and patient education/counseling on lifestyle modification and appropriate outpatient referrals for patients with Heart Failure, LVADs, cardiac diseased patients waiting on transplants or recently transplanted, post MI, irregular heart rhythms, and more, as well as the potential for additional co-morbidities.
Actively participates in Interdisciplinary Rounds to provide critical information regarding evaluations and treatments to plan for a safe discharge and closely collaborates with providers, Physical Therapy, Occupational Therapy, and Nursing Staff to provide safe care to patients.
Participates and develops interventions to meet fall safety goals in collaboration with unit leadership, serving as the unit fall liaison to provide education back to unit staff and patients regarding fall prevention and data review.
Performs ECG/EKG as needed.
