Catholic Health Physical Therapy and Rehabilitation in Port Jefferson, NY, gives patients access to the hospital's award-winning rehabilitation services in an outpatient facility.
St. Charles Hospital is the only hospital on Long Island accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF) for comprehensive inpatient rehabilitation and outpatient rehabilitation for adults and children.
Our Port Jefferson facility, located within St. Charles Hospital, offers one of the most comprehensive CARF-accredited services for outpatient rehabilitation found in a single location anywhere on Long Island. It is a premier resource for Long Islanders who need speech, occupational and physical therapy. Our patients have convenient access to the hospital's radiology and laboratory testing as well as access to a team of physiatrists and other specialty physicians.
Requirements:
- NY State RN license and Registered NYS Nurse Practitioner license, and adult, family or geriatric certification.
- BLS certification required, as is critical thinking abilities and superior interpersonal and technical skills
- NP/PA experienced required
Responsibilities:
- Provide comprehensive care to assigned patients along with attending physician
- Providing medical interventions as indicated
- Monitoring laboratory and test results
- Obtaining appropriate consultations as needed
- Assist with patient discharges/discharge planning
- Monitoring patient progress in rehabilitation for safe discharge to home or other facilities
- Demonstrating care that reflects an understanding of individual cultural and religious diversities and needs, along with the recognition of patient rights.
- Thoroughness of patient care
- Medical and clinical knowledge especially in PM&R, clinical judgment and technical skills, utilize current “best practices”
- Ability to work with others, establish rapport with patient and families
- Maintain patient confidentiality and demonstrate ethical standards in treatment
- Commitment to continuous personal development
- Clarity and legibility of medical records - timely and completeness of documentation
- Advocate for quality patient care
- Assist patients dealing with system complexities
Salary Range
USD $66.55 - USD $90.00 /Hr.
