The role you’ll contribute:
GENERAL SUMMARY:
The RN Extender is responsible for the delivery of coordinated nursing care for a patient and/or assigned group of patients within a patient care unit and for the day-to-day management and supervision of the related clinical activities. Under the direction of a Registered Nurse (RN), the RN Extender functions to provide and ensure the completion of activities of various levels of assigned nursing and ancillary personnel for that patient or group of patients. The RN Extender practices within the State, Facility and regulatory guidelines.
PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:
The RN Extender is responsible for execution of the Nursing Process under the supervision of the RN:
Assessment –collects patient health data
Data collection involves the patient, family, and other health care providers as appropriate.
The priority of data collection activities is determined by the patient’s immediate condition or needs.
Pertinent data are collected using appropriate assessment techniques and instruments.
Relevant data are documented in a retrievable form.
The data collection process is systematic and ongoing.
Diagnosis – the assessment data and diagnoses derived from the RN. Diagnoses are derived from the assessment data.
Diagnoses are validated with the patient, family, and other health care providers, when possible and appropriate.
Diagnoses are documented in a manner that facilitates the determination of expected outcomes and plan of care.
Outcome Identification –identifies expected outcomes individualized to the patient.
Outcomes are derived from the diagnoses.
Outcomes are mutually formulated with the patient, family, and other health care providers, when possible and appropriate.
Outcomes are culturally appropriate and realistic in relation to the patient’s present and potential capabilities.
Outcomes are attainable in relation to resources available to the patient.
Outcomes include a time estimate for attainment, provide direction for continuity of care, and are documented as measurable goals.
Planning – works with the RN to develop a plan of care that prescribes interventions to attain expected outcomes.
The plan is individualized to the patient (e.g., age-appropriate, culturally sensitive) and the patient’s condition or needs.
The plan is developed with the patient, family, and other health care providers, as appropriate.
The plan reflects current nursing practice.
The plan provides for continuity of care.
Priorities for care are established.
The plan is documented.
Implementation –implements the interventions identified in the plan of care.
Interventions are consistent with the established plan of care.
Interventions are implemented in a safe, timely, and appropriate manner.
Interventions are documented.
Evaluations –evaluates the patient’s progress toward attainment of outcomes.
Evaluation is systematic, ongoing, and criterion-based.
The patient, family, and other health care providers are involved in the evaluation process, as appropriate.
Ongoing assessment data are used to revise diagnoses, outcomes, and the plan of care, as needed.
Revisions in diagnoses, outcomes, and the plan of care are documented.
The effectiveness of interventions is evaluated in relation to outcomes.
The patient’s responses to interventions are documented.
The RN Extender practices in accordance to the established Georgia Nurse Practice Act and law.
The RN Extender is responsible for maintaining general mandatories established by Gordon Hospital and any unit-specific mandatories, as deemed necessary for practice within that setting.
The RN Extender is responsible for legal, ethical practice.The nurse serves as a patient advocate in accordance with the patient bill of rights.The nurse is responsible for compliance with regulatory standards pertinent to nursing practice.
The RN Extender must remain flexible to changing systems, is expected to demonstrate quality and effectiveness in work habits and treats staff, physician, patients and families with considerationandrespect.
Skills
Administers all types of medications correctly per age group for all populations served according to policy
Demonstrates proficient technical skills and operational knowledge of all equipment used on the unit.
Performs daily rotational task assignment as directed.
Supervises Nursing Assistants in the process of basic nursing functions (i.e. bed making, toileting, oral care, hair and nail care, baths and comfort measures in accordance with the patient’s cultural needs).Demonstrates standard procedure for safe patient ambulation and transfers, and checking, turning, and repositioning bedfast, and or, incontinent patients every two hours.
Communication/Collaboration
Gather data/assess/reassess all populations served and assist with primary assessment of all newly admitted or transferred patients and review care plans for each patient. Assessments/re-assessments/care plans must be validated by a registered nurse.
Documents/communicates clearly and concisely patient data, initiates and implements phone and verbal physician orders promptly and accurately as needed, collaborates with other members of the healthcare team as needed including the Charge RN, other RNs, and physicians, under HIPAA and confidentiality policy.
Collaborates with Charge RN on Care Plan input
Timely collaboration with RNs to establish priorities in planning, organizing, and implementing patient care based on nursing assessment, and taking into consideration the patient/family’s psychosocial/cultural/circumstantial situations.Timely collaboration with RNs to evaluate patient response to nursing interventions.
Demonstrates recognition of emergency codes, infection control, life safety, and fire plan, age specific safety concerns, proper body mechanics and ergonomics, and implements measures to meet these needs when they arise in collaboration with the RN.Supervises nursing assistant staff during these events.
Understands the Computerized Physician Order Entry Process and proficiently uses the electronic medical record for retrieval of patient information.
Monitors Electronic Medical Record for new physician orders and diagnostic results.
Provides the necessary and appropriate education to the patient and family from admission through discharge. This includes, but is not limited to: the patients known medical and nursing plan of care, medications, tests and procedures, disease process, and discharge instructions. Initiates and documents the education provided including the patient response and outcomes. Initiates referrals to healthcare providers as necessary.
Qualifications
EDUCATION AND EXPERIENCE REQUIRED:
Graduate of accredited LPN program.
EDUCATION AND EXPERIENCE PREFERRED:
Acute care experience preferred
3 years work experience practicing as LPN
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
Medical Surgical Unit
Active Georgia LPN license upon hire
Basic Life Support (BLS) certification upon hire
Advanced Cardiac Life Support (ACLS) certification within 1 year of hire
Pediatric Advanced Life Support (PALS) certification within 18 months of hire.
Successful completion of the LPN Credentialing Course within 90 days of hire.
Completion of RN program and RN licensure within 5 years of hire.
