5 West - Oasis Unit5W Oasis Transitional Care Unit (OTCU) is a 14-bed inpatient unit which provides care for patients with complex discharge barriers. Oasis has a diverse, interdisciplinary team to creatively address these barriers in an innovative, holistic care environment. Quality treatment for patients with multiple medical and psychosocial needs is provided by integrating nursing care and psycho-social assessment. This team facilitates meaningful, patient activities such as music, art and wellness programs. Each patient is treated with dignity and their strengths are identified in the highly personalized care provided. The Oasis team is high-performing, and with their innovative approach, they make a positive impact on patient outcomes. This nursing team is highly engaged and consistently ranks high in annual employee engagement surveys. The 5W Oasis nursing care model consists of an RN/LPN/CNA partnership. The nurse-to-patient ratio (RN/LPN) are 1:4 - 1:5. CNA staffing ratios are 1:7.
Job Summary
The Activity Coordinator is responsible for assisting with planning, organizing, implementing, and leading the activities for Oasis Transitional Care Unit in order to enhance the patient's independence and quality of life. We are looking for a fun and creative personality that will grow the program and foster a sense of community and social engagement. This program is designed in accordance with the comprehensive assessment, the interests, and the physical, mental, spiritual and psychosocial well-being of each patient.
Essential Functions:
Develop, implement, schedule, manage and provide therapeutic activities (including evening and weekend activities) for the OTCU patients. (30%)
Communicate directly to nursing staff, and nurse program manager, regarding patient?s observed actions that might be exceptional either positively or negatively for the patient. (15%)
Coordinate with other staff to schedule events and use of facilities. Schedule and posts all activity calendars on OTCU and in each of the patient's rooms. (15%)
Promotes positive patient experience (10%)
Identify items needed to provide scheduled activities, store, organize, gather materials, and implement activity programs planned. (10%)
As a team, develop and implement monthly engagement calendars, per resident interests and choices. (10%)
Conduct evening and weekend activities independently according to the activity calendar (5%)
Prepare monthly activity calendars, promotional flyer's, and information materials (5%)
Education:
High School Diploma or GED Required
Work Experience:
1-3 years of Activities Coordinator, or related Coordinator experience, Required

Must be detail oriented and able to problem solve minor issues.
