JOB SUMMARY
This position provides psychosocial assessments, diagnosis, treatment, discharge planning and consultation to patients and families coping with hospitalization, illness, diagnosis, treatment, and emotional, mental, and substance use disorders. Responsibilities include, but are not limited to provision of direct and indirect patient care, care coordination and case management, ability to provide social work perspective within multidisciplinary care teams, and other related duties as assigned.
JOB DUTIES AND RESPONSIBILITIES
Provide psychosocial assessments, diagnosis, and treatment plans using evidence-based interventions.
Deliver individual and group psychotherapy.
Provide discharge planning and consultation services for patients and their families, working in coordination with the healthcare team to ensure a smooth and effective transition of care.
Respond to patient inquiries and concerns via telephone or EPIC MyChart messages and be accessible to consult with supervising provider as needed.
Provide timely documentation in the electronic medical record of all relevant data, in accordance with the Department, Medical Center, and Health System policies.
Assist with clinical case assignments to psychiatry residents and trainees within Civitan's adult service program.
Participate In multidisciplinary meetings and represent the clinical social work perspective.
JOB ELIGIBILITY REQUIREMENTS
Education
Level I and Level II
Master's degree in social work from an accredited school of social work .
Experience
Level I
Two year of post internship social work experience in a health care setting or social service agency. Two years recent post master's experience in a clinical setting preferred.
Level II
Same as Level I plus five years post master's experience in a health care or social service agency. Experience in leadership and supervising.
Degrees, Licensure, and/or Certification
Level I and Level II
Current licensure as a licensed clinical social worker (LCSW) by the NC Social Work Certification and Licensure Board.
