Location:This is a Home Health Fieldwork position doing patient home visits primarily in the Volusia area as needed.
The role you’ll contribute:
Primarily responsible to carry out the treatment plan established by the Physical Therapist for the rehabilitation of patients who have impairments, functional limitations, disabilities or changes in physical function and health status. Intervention is purposeful and skilled utilizing the therapeutic application of physical and chemical agents, exercise and other procedures to produce changes in the condition consistent with the diagnosis and prognosis. Interact and collaborate with the patient and family as well as the clinical team
The value you’ll bring to the team:
Provides physical therapy intervention based on the plan of care. Provides therapeutic exercises to develop strength and endurance, range of motion and flexibility; therapeutic activity to improve functional performance; neuromuscular re-education to improve balance, coordination, kinesthetic sense, posture, and proprioception; gait training to instruct in the use and care of orthosis, prosthesis and assistive devices; wheelchair management to facilitate proper positioning and mobility.
Provides modalities including ultrasound, electrical stimulations, hot/cold packs, mechanical traction, TENS application as ordered to reduce pain and promote mobility.
Instructs the patient and caregiver in care and use of physical therapy devices and the patient’s total physical therapy program including the home exercise program.
Observes and records activities and findings in the clinical record and reports to the physical therapist and/or physician the patient’s reaction to treatment and any changes in patient’s condition, or when there are deviations from the plan of care.
Adheres to the plan of care, demonstrates competency in all procedures within scope of practice as appropriate to the age of the patient served. Applies or modifies skilled intervention (within scope of practice) to the patient using various procedures and techniques to achieve the best outcome possible for the patient. Assesses patient response to interventions and reports appropriately to the supervising therapist.
Completes thorough clinical documentation that accurately reflects treatment provided consistent with insurance guidelines/requirements and acceptable practice standards.
Minimum Qualifications:
Associate degree in Physical Therapy Assistant with at least one year of experience under the supervision of a licensed Physical Therapist.
Graduated from an approved physical therapy curriculum as required by the state which issued the license
Current state of Florida Physical Therapist Assistant License
Valid Driver’s License and current car insurance
BLS certified
Knowledge and Skills required:
Must be fluent in the English language
Demonstrates moderate computer skills
Education and Experience preferred:
At least 2 years of clinical experience as a physical therapist assistant
Experience working with adult and geriatric rehabilitation cases
Home health experience within the past 3 years
Current knowledge of homecare requirements
Previous home health experience
Knowledge of regulatory requirements
